What the TSA Actually Allows in Your Carry-On

Insulin, pens, needles, lancets, test strips, and continuous glucose monitor supplies all clear security, and they do not count against your liquid limit. The rule that matters is simpler than most people expect: keep diabetes supplies in your carry-on, not your checked bag. Cargo holds on some regional aircraft are not temperature-controlled, and insulin that freezes or bakes loses potency.

You are not required to carry a doctor’s note, but a printed prescription label on the box speeds things up if a checkpoint officer has questions. Insulin vials and pens must have the pharmacy label attached. Ice packs to keep insulin cold are allowed as long as they are frozen solid at the checkpoint; once they start to melt, they fall under the 3.4-ounce liquid rule.

Ask for a pat-down instead of the body scanner if you wear a CGM or insulin pump and prefer not to disconnect. Officers are trained for this, and it is a request, not a negotiation. Build ten extra minutes into your airport arrival the first time you fly with new equipment.

Packing the Kit You Will Actually Use at 35,000 Feet

Double the supplies, halve the assumptions

Pack twice the test strips, sensors, and insulin you expect to need for the trip. Delays and diversions happen, and a replacement sensor is not something you can buy at a Hudson News. A reasonable rule for a domestic round trip is one full backup vial or pen plus a spare of every device you depend on.

Snacks belong in the same bag. Airport food courts run heavy on carbs and light on predictable timing, so carry fast sugar for lows and something with protein and fiber for the hours when a meal gets pushed. A small cooler pouch, a glucose meter with fresh batteries, and a written list of your medications and doses round out the kit. Keep the medication list on paper, not just in your phone, since you may need it if you end up at an urgent care in another state.

If you use a CGM, pack an extra sensor and confirm your phone app works offline. Some readers alarm only through the app, and airplane mode with no Wi-Fi can silence the alerts you rely on. Test the setup at home before you fly.

Timing Doses Across Time Zones

Flying east shortens your day; flying west stretches it. For most people on metformin or a once-daily basal insulin, the practical fix is to keep doses roughly twelve hours apart and shift gradually toward the new local time over the first two days. A three-hour time difference usually needs no adjustment at all.

Anything more complicated — mealtime insulin, a GLP-1 medication like semaglutide, or an injectable taken weekly — is worth a five-minute call to your prescriber before you book. Weekly injections are the easiest to plan: pick a day and stick to it regardless of time zone. Ask specifically whether your maintenance dose of semaglutide needs any change for travel; for most people it does not, but the answer should come from your clinician, not a forum.

Check whether your metformin dose is doing what you think it is. A 500 mg tablet is a low dose, and some people flying with a stale regimen find their numbers drifting higher than expected on travel days. Bring your log so you can spot the pattern rather than guessing at the gate.

Managing Blood Sugar During the Flight

Cabin pressure, long stretches of sitting, and disrupted sleep all push glucose upward. Stand and walk the aisle every hour on flights over three hours, and drink water steadily — dehydration makes readings look worse than they are. Test on your normal schedule rather than waiting for symptoms.

Carry your own food if your flight crosses a mealtime. Domestic carriers rarely stock anything suitable for a person counting carbohydrates, and pretzels and cookies are not a plan. A sandwich, nuts, and a piece of fruit in your bag cover most four-hour flights.

Know where your glucose is before boarding, and do not board with a reading heading down and no fast sugar in reach. If you use insulin and your glucose runs low during boarding, treat it first and let the crew know; gate agents can hold the door for a few minutes. Hypoglycemia at altitude is uncomfortable but manageable when you have glucose tablets in your pocket instead of in the overhead bin.

Finally, wear or carry identification that says you have diabetes. A medical ID bracelet or a card in your wallet gives a stranger the one piece of information that matters in an emergency. It costs under $40 and takes a week to arrive, so order it before the trip, not after.
